A Line Operations Safety Audit (LOSA) is a formal process that requires expert and highly trained observers to observe operations in real-time and to collect safety-related data on environmental conditions, operational complexity, and human performance. LOSA observer training is critical to the process.
LOSA is seen as an important way to help develop countermeasures to operational errors. It involves a structured program of observation of front line activities built around the Threat and Error Management (TEM) concept. It aims to identify threats to operational safety, identify and minimize the risks which are the origin of such threats, and implement measures to manage the human error aspects of the residual risk.
